Participant Waiver & Liability Release
Every participant must complete and sign this form before joining a SNUBA® Bonaire dive. You can download and pre-fill it at home or sign on the day of your experience.
SNUBA® Participant Record & Liability Release
How to complete your waiver
Download the PDF using the button above and print it at home — or we’ll have copies ready at Nautico Marina.
Fill in your personal details: full name, date of birth, address, phone, email and emergency contact.
Answer the medical questionnaire honestly. Questions 1–8 are absolute exclusions; questions 9–19 require physician clearance if answered yes.
Sign the liability release on page 2. If the participant is under 18, a parent or legal guardian must also sign.
Bring the completed form to Nautico Marina on the day of your experience, or hand it to your SNUBA® guide on board the Mushi Mushi.
Completing the waiver in advance saves time on the day and lets you board the boat without delay. We recommend downloading and printing it the night before your tour.
About the medical questions
The waiver includes 19 medical questions in two groups. Please answer every question honestly — your safety depends on it.
A YES answer to any of these means you cannot participate in the SNUBA® dive.
- Are you pregnant or do you believe you might be pregnant?
- Do you have a history of heart attacks, strokes or heart disease?
- Have you ever had heart surgery, angina or blood vessel surgery?
- Do you have asthma and are currently using an inhaler, or have a history of emphysema or tuberculosis?
- Are you currently, or within the past 8 hours, been under the influence of mind-altering drugs or alcohol?
- Do you have any form of lung disease?
- Do you have epilepsy, seizures or convulsions or take medications to prevent them?
- Are you actively taking medication that carries a warning about impairment of physical or mental behaviour?
A YES answer does not automatically disqualify you, but you must be cleared by a physician before diving.
- Do you have a history of blackouts, fainting or breathlessness?
- Do you currently have a head cold, sinusitis, bronchitis or ear discharge/infection?
- Do you have a history of diabetes affecting your ability to participate in strenuous activity?
- Do you have a history of asthma or wheezing with breathing or exercise?
- Have you ever had a diving accident or decompression sickness?
- Do you have high blood pressure or take medicine to control it?
- Do you have a history of bleeding or blood disorders?
- Have you ever had ear or sinus surgery?
- Do you have a history of ear disease, hearing loss or problems with balance?
- Do you have problems equalising ears with airplane or mountain travel?
- Have you ever had a brain, spinal cord or nervous disorder?
If you’re unsure about any of the above, please contact us before booking. We’re happy to answer your questions and help you determine whether SNUBA® is right for you.
About the liability release
Page 2 of the waiver contains the Liability Release & Express and Primary Assumption of Risk form, issued by SNUBA® International, Inc. By signing, you acknowledge the inherent risks of the recreational sport of SNUBA® and agree to release the listed parties from liability.
“I hereby affirm that I have been advised and informed of the inherent risks and hazards of the recreational sport of SNUBA®, including but not limited to dangers associated with breath-holding, rapid ascents, and lung over-expansion, as well as water-related injuries such as death by drowning… I still choose to proceed in this Adventure despite the inherent risks and hazards.”
Important: By signing, you also agree that any legal claims will be adjudicated under California law (as per SNUBA® International terms), and must be brought within one year of the date of the incident.
The form also includes a section about promotional photography: SNUBA® International may use photos or videos of your experience for promotional purposes. You can opt out by checking the box on the form.
Minors & parental consent
Children from 8 years old are welcome to SNUBA® Bonaire. For participants under 18, both pages of the waiver require the signature of a parent or legal guardian in addition to the participant’s own signature where applicable.
We recommend that parents complete the medical questionnaire on behalf of younger children and be present at check-in to sign the form.
